Chronic condition care

Chronic disease clinics are run by the Practice Nurse Team. These clinics are designed to monitor patients with the following conditions:

  • Asthma
  • Hypertension
  • Diabetes
  • Coronary heart disease
  • COPD
  • Blood Pressure/Hypertension

All patients with these conditions will be invited for an annual review. We strongly recommend that you attend this review. Patients with diabetes or high blood pressure should be seen at least every six months; we encourage you to be responsible for booking and keeping these appointments with us.
