Dementia advice

We have the services of an Alzheimer’s Support Dementia Adviser who is based in the PCN every Tuesday and Friday.

Our Dementia Adviser is available to provide up to date information and support to people living with dementia and their families. She will help Alzheimer’s sufferers and patients with memory loss to access the services they need. The dementia adviser works closely with our GPs. She will help and advise carers and patients with a current diagnosis of dementia and those who are worried about or living with memory loss. You can get in touch with her direct on 07702285509.
