We will assist people who have a disability, impairment or sensory loss, to ensure equality in access to care.
Patients or their carers should alert the Practice of any disability or additional support required and give details of how these needs can be met. This may be done at the point of registration (for new patients) or at any point during your care journey. Details of any support needs will be added to the patient record so that all Practice staff, and other NHS and Social Care professionals involved in your care, are aware.
The Practice works to ensure that patients and service users, carers and parents, can access and understand the information they are given.
Patients should inform the Practice if they require information in a different format, for example in large print, braille, easy read or via email. The Practice will endeavour to ensure that people get any support with communication that they need, for example from a British Sign Language (BSL) interpreter, deafblind manual interpreter or an advocate.
Guide Dogs / Assistance Dogs / Hearing Dogs Policy
The Practice welcomes assistance dogs. Assistance dogs must wear a special identifying harness and collar tag.
Owners of assistance dogs will be given the opportunity to tour the public/patient areas of the Practice and the grounds with their assistance dog. This will enable the dog to become familiar with routes throughout the building.
Assistance dogs must always remain on a lead and in close contact with the owner.
The Practice welcomes feedback from patients regarding disability access and, wherever possible, will make reasonable adjustments.
